| PUBLIC_BASE_URL | No | The public base URL for the extension. Leave blank to use the default. | |
| FORM_DESCRIPTION | Yes | The description of the form (e.g. Open this form to display the Hello World MCP App POC.). | |
| FORM_SESSION_FIELD | No | For flow or router forms, the hidden session field. Leave blank for standalone forms. | |
| AUTH0_FORMS_TRUST_SECRET | No | The trusted-session secret configured for Forms. Leave blank for standalone forms. |
